INFORMATION ON THE PROCESSING OF PERSONAL DATA pursuant to Czech Personal Data Protection Act, Act No. 101/2000 Coll. on the Protection of Personal Data, as amended (hereinafter referred to as "the Act") and Regulation (EU) 2016/679 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (General Data Protection Regulation, hereinafter referred to as "GDPR")

The personal data of Client (name, telephone, address), including special categories of personal data (in particular data regarding health status and data indicative of race or ethnic origin, political opinion, religious affiliation, philosophical beliefs, and sexual orientation), are processed by the controller in accordance with the provisions of Article 6(1)(b) GDPR for the purpose of fulfilling the Agreement for Provision of Psychotherapy, concluded between Client (data subject) and Coordinator (data controller), in accordance with the provisions of Art. 6 para. 1 lit. a) for the purpose of providing psychotherapeutic consultations and in accordance with Art. 6 para. 1 lit. f) for the purpose of protecting the legitimate interests of the controller in the event of a contractual dispute, unless the interests of Client outweigh the interests of the controller in a particular case.
The recipient of Client's personal data is Coordinator and, based on Client's consent in exceptional cases, also persons cooperating with the controller, exclusively in the interest of Client and for the purpose of fulfilling the contract concluded between Client and the controller.
Coordinator as a data controller processes the personal data of Client on the basis of the Agreement concluded with Client. The provision of personal data is a prerequisite for the relevant provision of services by the controller to Client. The data controller keeps the personal data of Cient for the duration of the Agreement and for three (3) years after the end of the Agreement.
Client acknowledges that, in accordance with the aforementioned legal provisions (Act and GDPR), Client is entitled as follows:
- To withdraw consent at any time, for each of the purposes of processing separately,
- To request information on what personal data is processed about him/her,
- To request correction or completion of his/her personal data,
- To request deletion of personal data for which there is no longer a reason for processing,
- To request restriction of processing data that are inaccurate, incomplete or for which the reason for processing has ceased to exist, but for which he/she does not consent to their erasure,
- To request the transfer of the data processed,
- To receive a reply to his/her request addressed to the controller without undue delay, in any case within one month of receipt of the request by the controller,
- To lodge a complaint with the supervisory authority (Office for Personal Data Protection, pplk. Sochora 27, 170 00 Prague 7, Czech Republic) if he/she considers that the processing of his/her personal data violates the GDPR.
